Conduct comprehensive assessments of patients' health status and symptoms
Develop and implement individualized care plans in collaboration with other healthcare providers
Manage medications and treatments to alleviate pain and other distressing symptoms
Provide emotional and psychological support to patients and their families
Educate patients and families about illness progression and available treatment options
Coordinate with social workers, chaplains, and other healthcare professionals to ensure holistic care
Facilitate advanced care planning and end-of-life discussions
Monitor and evaluate patient progress, adjusting care plans as needed
Participate in interdisciplinary team meetings to review and update patient care plans
Advocate for patients' needs and preferences within the healthcare system
Conduct and contribute to research in palliative care practices and outcomesRequirements:
Minimum of 2 years of experience in a palliative care setting
Master's degree in nursing, with a specialization in palliative or hospice care
Experience in managing chronic pain and symptoms related to serious illnesses
Experience working within a multidisciplinary healthcare team
Familiarity with electronic health records and palliative care documentation
Training in advanced care planning and end-of-life care discussions
Active nurse practitioner (NP) license and DEA for Washington state
Strong clinical assessment and diagnostic skills
Demonstrated ability to manage complex patient cases
Proficiency in patient education and counseling

Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $138,000 to $164,000 annually with no cap on productivity income potential. This full-time role is benefit eligible.
