What will I do as the Assistant Director of Patient Services?

Primary functions are to support the Director of Patient Services (DPS) in the CHHA and LHCSA, as well as supervising, instructing, and guiding clinical personnel in providing high quality clinical services. Responsibilities will be carried out in accordance with the philosophy, objectives and directives of the Agency. Promotes and restores patients' health by developing day-to-day management and long-term planning of CHHA/LHCSA home care patients; coordinates with physicians; staff; and DPS.

Maintains knowledge of Federal State and local regulatory requirements.

Assists in the maintenance and development of agency policies and procedures to ensure quality and safe delivery of care.

Ensures compliance with policies and procedures regarding operations of the Agency. Assures quality of care by developing and interpreting standards of care; enforcing adherence to State Board of Nursing and State Nurse Practice Act requirements and to other governing agency regulations; measuring health outcomes against standards; making or recommending adjustments.

Participates in the recruitment/interviewing/selection/orientation of team members, evaluates their probationary and annual performance relative to job goals/requirements, coaches staff recommending in-service educating programs. As well as providing continual supervision, compliance, and supervisory visits.

Reviews referrals for appropriateness, directs care as ordered by physician, coordinates nursing visits, reviews plans of care and OASIS to ensure compliancy, appropriateness and completeness, provides clinical oversight, case conferences, and discharge planning.

Works collaboratively with staff members, community agencies, and resources affiliated with Acacia Home Care to promote effective communication, demonstrates skill in conflict resolution and encourages a positive work ethic.

Promotes continuity of care through discharge planning, case management, direct care, referrals, advocacy, and ongoing case conferencing with staff.

Performs clinical record reviews in collaboration with the Quality Improvement Team reports findings, develops and implements a plan of correction for identified areas.

Attends QAP/Quality improvement meetings. Abides by the Compliance Program at Masonic Care Community ensures that all information is kept confidential and is respectful of the Patients' Bill of Rights.

Participates in promoting awareness of compliance in staff and by assisting with monitoring activities and follow up actions.

Performs essential functions of Registered Nurse and Home Health Aides in the field, as staffing needs arise. Will rotate on call coverage with the DPS routinely and in the DPS absence.

Qualifications:

Registered Nurse with a current New York State License who is responsible for clinical direction and supervision of patient care services.

Two years of home care experience with Associates RN Degree or one year of home care experience with BSN RN Degree and Graduate of an accredited School of Nursing.

One year of full-time experience in a supervisory or administrative capacity in nursing required.

Valid New York State Driver's License or access to transportation.
