Registered Nurse (RN) Case Manager

No On Call shifts - We have a dedicated After Hours and Weekends Team!

A reasonable caseload that allows you to provide exceptional, high quality care while maintaining a good work life balance

Hours: Monday-Friday 8:00 a.m. -5:00 p.m.

Holiday requirement, 1 major holiday and 1 minor holiday per year

Opportunity for additional shifts

Expected to make up to 5 visits per day with alotted time to complete patient documentation

Highly autonomous and functions with minimal supervision

Weekly interdisciplinary meetings

24/7 support from our full-time Board Certified physicians and APRNs, Chaplains and Social Workers

Interdisciplinary team work to support outstanding patient and family care

Position Description:

We are seeking a compassionate Registered Nurse (RN) to join our interdisciplinary hospice team in providing high quality, expert care to our seriously ill patients. Our RN Case Manager is a vital part of our hospice team providing comfort, care, support and education to our patients and their loved ones. This RN Case Manager will see patients primarily in the home setting.

The Ideal Candidate Will Possess the Following Requirements:

Valid, unencumbered Missouri and Kansas RN or compact license to include both states

Minimum of one year bedside experience in caring for hospice patients is preferred

Ability to communicate effectively and professionally with others; and to prepare and present information in oral and written formats

Superior organizational skills with the ability to multitask and meet deadlines; must schedule and manage time effectively

Valid driver's license and insurance plus reliable transportation

CPR certification required
