RN Client Care Coordinator

Job Description

Comfort Keepers is a home care agency, locally owned and operated since 2004. We provide homemaking, home health aide, skilled nursing services in Central Minnesota. We have offices in St. Cloud, Nisswa, Litchfield and Buffalo.

We are currently looking to hire a full-time RN for our growing company. We are non-medicaid and non-medicare.

Job duties and tasks may include:

-Case Management

-Skilled Nurse Visits

-Triage

-Full Intake/Open process that includes assessments, contracts, and computer entry.

-Training and oversite of non-licensed staff.

-Supervised visits.

-Re-assessments.

-Communications with referral sources, medical providers, clients, and family.

-Work environment includes office, client homes through our Central Minnesota territory, and working on the road between clients.

40 Hour Work Week: Monday-Friday 8am-430pm

Pay: $36.00 - $40.00 per hour
