Our Community Living Supports (CLS) program provides in-home and community-based services that promote autonomy, inclusion, and dignity for individuals across Macomb County.
The Program Manager – Community Living Supports (CLS) is responsible for overseeing the delivery, quality, and compliance of CLS services for individuals with developmental disabilities. This role includes supervising direct support staff, coordinating services based on Person-Centered Plans (PCPs), ensuring compliance with Macomb County Community Mental Health (MCCMH) and Medicaid standards, and serving as a liaison between clients, families, and stakeholders.
Minimum Level of Education, Training, and Experience
At least 3 years of experience working with individuals with developmental disabilities in a residential setting.
Minimum of 1–2 years of supervisory or program management experience, ideally within a human services setting.
Strong knowledge of Michigan Medicaid CLS services, MCCMH standards and Person-Centered Planning.
Experience with behavioral health documentation and compliance.
Strong leadership, organizational, and communication skills.
Ability to manage multiple priorities in a fast-paced environment with compassion and professionalism.
Proficient in electronic documentation, Microsoft Office, and AFC documentation standards.
Valid Michigan driver’s license and reliable transportation; local travel required.
Preferred: Familiarity with HCBS (Home and Community-Based Services) compliance and Rights protections in Michigan.
Principle Duties and Responsibilities
Manage day-to-day operations of CLS programming, including staffing, service delivery, and scheduling.
Supervise and support CLS Coordinators and Direct Support Professionals (DSPs); conduct performance evaluations and ensure staff training is current.
Participate in incident reporting, licensing reviews, and corrective action plans.
Ensure services reflect PCP goals and are tailored to each client’s unique needs, strengths, and preferences.
Serve as the primary contact for MCCMH case managers, families, guardians, and other service providers.
Conduct regular in-home visits to monitor quality of care and ensure participant safety and satisfaction.
Oversee documentation of service delivery, incident reports, and staff activity logs in accordance with Medicaid and MCCMH policies.
Monitor service utilization and billing to ensure accuracy and adherence to authorized hours.
Ensure compliance with Medicaid rules, CLS provider requirements, Recipient Rights policies, and HIPAA confidentiality standards.
Support the development and implementation of behavioral plans in coordination with behaviorists and clinicians.
Recruit, onboard, and retain qualified staff to meet growing service needs.
Participate in audits, licensing reviews, and continuous quality improvement initiatives.
Working Conditions
Mix of office and field work; regular travel to AFC homes across Macomb County.
May require occasional evening or weekend hours based on program needs.
Ability to respond to emergencies or licensing visits on short notice.
