Job Description
Schedule: Full time, hourly, non-exempt - Tuesday-Friday 3pm-11pm (32 hours)
This position provides direct nursing care to patients in the IPU, their families, interdisciplinary group management, coordination of patient and family services, and evaluation of appropriate hospice care through development of the Hospice plan of care.
What are the essential duties day to day?
Symptom management
Patient Assessment
Documentation - assessments, interventions, and updates
Develop and coordinate plan of care
Educating patients and family members
Communicate with our medical providers onsite
Average of 6 patients a day
Supporting other staff members and team members
Training members
Supervise CNA services provided to assigned patients

In hospice care, you are dealing with grief. It is important to find ways to not take the heaviness of it all home with you.
