Hospice RN On-Call

Job Description

Pathway Hospice, the leading locally owned hospice provider in DFW, is seeking an experienced RN to join our clinical team as a Hospice On Call Registered Nurse. Our ideal candidate will provide exceptional care and service for Pathway's patients and families.

Responsibilities of this position include the following:

Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME

Develops, coordinates, and follows the patient's individualized Plan of Care

Coordinate with team members/physicians and participate in staff and Interdisciplinary Team meetings

Appropriately identify and utilize resources: Dietary, Pharmacy, Supplies, Home Medical Equipment, etc.

Serve as a patient and family advocate to ensure delivery of the highest quality of care, providing education to patients, families and caregivers

Complete documentation in a timely manner

Communicate effectively with coworkers and patients/family

Participate in bereavement support
