Job Description
We are seeking Full-Time and Part-Time Community Participation Support Professionals in Philadelphia, PA. The Community Participation Support is a program designed to help individuals actively engage in their communities and develop skills for greater independence and inclusion. It focuses on providing opportunities and support for individuals to participate in a variety of community activities, both independently and with assistance. CPS aims to integrate individuals into their community life, build their social networks, and enhance their skills related to employment and community involvement.
Community Participation Support Professional (CPS)/ Direct Support Professional will provide caregiving assistance in areas of daily living for adults with mental health/intellectual disability diagnoses
Community Participation Support Professional (CPS)/ Direct Support Professional will collaborate with the treatment team to properly implement goals listed in Individual Support Plans (ISP)
Community Participation Support Professional (CPS)/ Direct Support Professional will transport or accompany participants to and from appointments
Community Participation Support Professional (CPS)/ Direct Support Professional will plan activities for the participant to engage in the community for the day. A list of available activities will be provided by the supervisor
Physical Demands
While performing the duties of this job, the staff is regularly required to talk or hear others and accurately interpret speech and sounds. Frequently required to stand; walk; climb and descend stairs. Required to use hands and arms to reach, hold, and carry objects. Required to operate a motor vehicle on a regular basis and assist participants when entering and exiting vehicles. Required to sit; stand, maintain balance; occasionally lift and/or move up to 50 pounds. Vision requirements include close vision, distance vision, color vision, peripheral vision, depth perception & ability to adjust optical focus for reading, driving, general observation, and for all safety needs.
Requirements
High School Diploma/GED
1-2 years MH/BH-related experience
My ODP Community Participation Supports training (KMS will provide the information, if needed)
A valid driver's license is required
