Position: Care Manager – Developmental Disabilities Services

Location: Fargo, North Dakota

Employment Type: Full-Time / Part-Time

What You'll Do:

As a Care Manager, you’ll play a vital role in supporting individuals with developmental disabilities in their daily lives. You will help with personal care, life skills, community engagement, and household responsibilities—all while following North Dakota DD Rules and Regulations to ensure safe, ethical, and person-centered care.

Key Responsibilities:

Provide respectful and compassionate support based on each individual's Person-Centered Plan (PCP).

Assist with daily living activities such as hygiene, meals, medication administration, and mobility.

Encourage independence, choice, and inclusion in the community.

Document all services and interactions per ND Department of Health and Human Services – DD Division standards.

Maintain confidentiality and uphold individual rights at all times.

Report changes, incidents, or concerns promptly in accordance with ND Administrative Code (75-03-23, 75-04-05, 75-04-06).

Participate in required training and ongoing professional development.

Follow emergency procedures and provide a safe environment.

What We’re Looking For:

High school diploma or GED.

Compassionate, patient, and dependable personality.

Ability to work independently and as part of a team.

Clear background check and valid driver’s license with reliable transportation.

Willingness to complete required DSP and DD training (paid).

Preferred (but not required):

Prior experience working with individuals with developmental disabilities.

Familiarity with North Dakota DD services and documentation systems such as Therap.
