Responsibilities
Guides and counsels individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
Assists clients in obtaining government funds, education, or treatment
Secures information, such as medical, psychological, and social factors contributing to client's situation, and evaluates these and client's capacities Asks clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem
Helps patient and family through individual or group conferences regarding plans for meeting needs, and aids client to mobilize inner capacities and environmental resources to improve social functioning
Helps client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and client's part in creating them
Develops service plan designed to restore patient to optimum social and health adjustment within patient's capacity
Refers clients to community resources and other organizations, and liaises with other social service agencies. Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability
Prepares patient histories, service plans, and reports. Reviews service plan and performs follow-up to determine quantity and quality of service provided client and status of client's case
Maintains client records in compliance with organizational goals and objectives, as well as external regulatory guidelines
Other duties as assigned
Qualifications
Master’s Degree in Social Work
Five plus years clinical social work experience
Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
Ability to make psycho-social assessments and develop and implement viable care plans
