Behavioral Health Professional - LCSW, LCPC, LMSW, LPC

We are seeking a licensed Occupational Therapist (OT) to join our home care team in a field-based role.

Provide client-centered OT services to help patients develop, recover, or maintain daily living and work skills.

Assess patients' functional status and home environment, identify hazards, and determine equipment needs.

Educate patients and caregivers on disease processes, medications, and personalized treatment plans.

Participate in developing and updating individualized care plans.

Maintain accurate documentation in compliance with agency standards using electronic medical records.

Support the organization’s mission by delivering high-quality home care with compassion and respect.

Qualifications include licensure, 1-2 years of clinical experience (home care preferred), reliable transportation, and basic iOS knowledge.
