RN Care Manager

The RN Care Manager serves Franklin County in a full-time, 40-hour/week role, providing end-of-life care with a focus on compassion and collaboration.

Responsibilities include assessing, planning, implementing, and evaluating patient care while working closely with families and team members.

Qualifications: RN license in Ohio, graduate of an accredited nursing school, preferred two years acute care experience, CPR certification, and computer skills.

Must have a valid driver’s license, pass background and drug screenings, and be able to drive safely.
