SOL PT - Physical Therapist

We seek a compassionate Nurse with a minimum of two years' experience in long-term care to join our team in Destin, FL.

Responsibilities include conducting resident assessments, ensuring timely and accurate documentation, and overseeing MDS processes in collaboration with the interdisciplinary team.

Must hold an RN/LPN license, have strong communication skills, and be able to make independent decisions.

Physical requirements include lifting up to 40 lbs and frequent movement throughout the day.

The role involves medication administration, discharge planning, staff training, and participating in survey processes.
