Class A Truck Driver

The Registered Nurse Case Manager plans, organizes, and directs hospice care, emphasizing community health education.

Conducts initial and ongoing comprehensive assessments of patients and families to determine needs.

Develops and revises personalized care plans involving the patient and family, administering medications and treatments as prescribed.

Provides health education, emotional support, and assists with activities of daily living to promote self-sufficiency.

Communicates effectively with physicians, caregivers, and interdisciplinary teams, documenting patient progress.

Participates in on-call duties, supervises ancillary staff, and contributes to quality improvement activities.

Requires nursing licensure, recent clinical experience, and valid driver’s license.
