Physical Therapist

Provides care management for specific patient populations by applying clinical expertise, communication, and problem-solving skills to optimize clinical and resource outcomes.

Promotes cost-effective, coordinated care and facilitates patient/family navigation through the healthcare system, including referrals and resource linkages.

Encourages interdisciplinary teamwork, ensuring continuity of care and effective communication across disciplines and transitions.

Engages and empowers patients/caregivers in disease management, assessing readiness for change and supporting lifestyle modifications.

Advocates for patients throughout their care episodes and participates in quality improvement initiatives and policy development.

Requires relevant education (RN or social work degree), licensure, and experience, along with strong interpersonal, analytic, and computer skills.
