Speech-Language Pathologist, PRN

The On-Call Registered Nurse organizes hospice care, responding to after-hours calls and visiting patients at home.

Performs comprehensive assessments, develops and revises care plans, and administers medications.

Provides patient and family education, emotional support, and self-care guidance.

Communicates with physicians and community providers to coordinate care.

Supervises hospice aides and ensures safety and continuity of care.

Maintains accurate documentation and participates in quality improvement activities.

Requires RN licensure, recent acute and home care experience, good communication skills, and a valid driver’s license.
