Seeking PRN Nurse Practitioners for home risk assessments, including new grads.

Conduct comprehensive adult/geriatric assessments in patients' homes, skilled facilities, and community settings.

Evaluate health status, medical history, medications, social factors, and identify care gaps.

Collaborate with physicians, social workers, and caregivers, providing follow-up and documentation.

Flexible schedule with daytime, evening, night, and weekend hours; travel required within Uniontown, PA.

Earn $2,000-$10,000/month based on completed assessments; benefits include mileage reimbursement, 401(k), and voluntary benefits.

Requirements: Master’s or equivalent, current RN/NP licensure, board certification, CPR/BLS, 1+ year NP experience, strong communication, and ability to travel.
