CDL-A Local Driver / Forklift Operator, Full-time

We are seeking a Nurse Practitioner (PRN) for home risk assessments, welcoming new grads.

Conduct comprehensive adult/geriatric health assessments in home, SNF, and community settings, reviewing medical history, social factors, and risks.

Collaborate with physicians, social workers, and caregivers to improve patient care and close care gaps.

Perform assessments lasting up to one hour, providing health education and follow-up.

Flexible scheduling including days, evenings, nights, and weekends, with travel required to patient locations.

Earn $1,800 to $9,000/month based on completed assessments.

Requires current RN and NP licensure, board certification, and at least 1 year of NP experience.

Must have valid driver license, reliable transportation, and computer skills.
