Nurse Practitioner

Seeking an Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to deliver comprehensive, person-centered medical care for long-term care and rehabilitation residents.

Salary range: $114,000-$125,000, based on experience.

Full-time, 40 hours/week, Monday-Friday days.

Collaborates with physicians and interdisciplinary teams to provide high-quality care, support, and education to residents and families.

Knowledge of palliative/end-of-life care and adherence to regulatory guidelines required.

Must have at least 2 years of experience, preferably in geriatrics or post-acute care.

Participates in on-call coverage, documentation, and quality improvement initiatives.

Supports residents' rights and serves on committees as needed.
