Avenues Recovery of Prince Frederick, MD is a unique community-based residential style program that's truly customized to the needs and unique circumstances of each individual.
Primary Focus: The primary function of the Clinical Intake Coordinator is to facilitate placement in treatment for those struggling with addiction by organizing and managing the process when an incoming patient reaches out to Avenues.
Responsibilities Include (but not limited to): Collaborate with and support admissions call center team by following up on patient inquiries, clarifying clinical criteria, sharing a sense of urgency to admit whenever clinically appropriate, maintaining relationships with referral sources as needed.
Maintain compassionate, professional, and confident rapport with patients to cultivate confidence in treatment and commitment to admit as scheduled.
Communicate program features, clinical impact, services offered, and any additional information needed to promote facility and support patients, families, and others instrumental in facilitating admissions for those who can benefit from treatment.
Involved in scheduling, transportation, and necessary arrangements to transition patients into treatment efficiently.
Facilitate proper communication is followed to ensure treatment team and support staff are prepared for admission, to orient client and address urgent clinical needs, and to follow up on anything needed to secure patients' placement at the facility and set client up for a successful treatment experience.
Coordinate with utilization review department for all case management, referrals, and authorizations needed prior to or immediately upon admit.
Clarify extended care plans in accordance with facility program framework and have approved plan and other special insurance needs such as FMLA/COBRA with executive director and confirm they are noted on client face sheet.
