Assignment Length: 13 Weeks

Schedule: Monday Friday, 8:00 AM 5:00 PM

Productivity Expectation: Minimum of 30 patients per week

Territory: 50-mile radius around the office

Job Description:

We are seeking an experienced and dedicated Registered Nurse (RN) to join our Home Health team in Roanoke, VA. In this role, you will provide, coordinate, and direct home nursing care in accordance with physicians orders and agency policies, using the full nursing process. You will play a key role in improving patient outcomes through individualized, in-home care.

Key Responsibilities:

Evaluate and assess patients' medical conditions and initiate the appropriate care plan

Regularly re-evaluate and update patient care plans as needed

Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses

Maintain accurate clinical documentation and communication

Collaborate with physicians and interdisciplinary team members

Educate patients and caregivers on care plans and health management

Utilize Homecare Homebase (HCHB) for electronic medical records

Qualifications:

Current and unencumbered RN license in Virginia or multi-state license under the Nurse Licensure Compact (NLC)

Minimum of 23 years of nursing experience (Home Health experience required)

Current CPR certification

Strong clinical assessment skills with an understanding of age-specific care needs

Proficient with general computer use and EMR systems

Bachelors degree in Nursing (BSN) preferred

Additional Information:

No weekend rotation or on-call required at this time
