Description:
This role involves working closely with patients, families, healthcare providers, healthcare facilities, and community resources to ensure optimal patient outcomes and quality of care - providing comprehensive care coordination and management for patients with mental health conditions. Market services to outside referral sources to increase incoming referrals and build relationships with referral sources. Must be willing to travel to market services to outside referral sources.
JOB DUTIES & RESPONSIBILITIES
Assess and evaluate patients' mental health needs, developing individualized care plans.
Coordinate and monitor patient care across various healthcare settings and providers.
Collaborate with interdisciplinary teams, including psychiatrists, psychologists, facility leadership, and social workers.
Provide patient and family education on care transitions.
Conduct regular follow-ups with patients to ensure adherence to treatment plans.
Facilitate smooth transitions between inpatient, outpatient, and community-based care.
Advocate for patients' needs and rights within the healthcare system and community.
Maintain accurate and up-to-date patient records and documentation.
Identify and connect patients with appropriate community resources and support services.
Participate in crisis intervention and management as needed.
Stay current with best practices and evidence-based treatments in mental health care.
Develop and implement marketing strategies to educate referral sources on care available at facilities.
Communicate with referral sources regularly to build strong relationships across the continuum of care.
Attendance to one’s entire shift is an essential function for the role.

Requirements:
POSITION QUALIFICATIONS
Education Associate Degree in Nursing – required
Bachelor Degree in Nursing – BSN – preferred
Experience
1 year of medical rehab and/or case management experience a plus
Strong knowledge of mental health disorders, treatments, and medications
Excellent communication and interpersonal skills
Proficiency in electronic health records (EHR) systems
Ability to work independently and as part of a multidisciplinary team
Demonstrated problem-solving and critical thinking skills
Compassionate and patient-centered approach to care
Certifications Current RN License in good standing in the State of Practice
Case management certification (e.g., CCM, CMAC) preferred
