The Advanced Practice Provider (APP)which may include a Nurse Practitioner (NP) or Physician Assistant (PA)provides high-quality, evidence-based care to residents in a nursing home or long-term care setting. The APP works collaboratively with physicians, nursing staff, and interdisciplinary teams to manage acute and chronic conditions, support preventive care, and ensure the overall well-being of geriatric patients in compliance with facility protocols and regulatory standards.
Key Responsibilities:
Conduct comprehensive assessments, including history and physical examinations, for new and existing residents.
Diagnose and manage acute and chronic illnesses commonly seen in geriatric populations (e.g., diabetes, heart failure, dementia, infections).
Prescribe medications and order diagnostic tests, therapies, and durable medical equipment as appropriate.
Monitor the effectiveness of treatment plans and make necessary adjustments.
Perform routine rounding and urgent visits, including post-hospitalization follow-ups.
Collaborate with physicians, nursing staff, therapists, social workers, and families to coordinate resident care plans.
Facilitate and participate in care conferences, advanced care planning, and end-of-life discussions.
Maintain accurate, timely documentation in the Electronic Health Record (EHR) in compliance with Medicare/Medicaid and facility standards.
Ensure regulatory compliance with CMS, state health departments, and infection control practices.
Provide on-call support as scheduled or required
Qualifications:
Licenses and Certifications Required
Current ND Board NP/PA License required.
Cardio Pulmonary Resuscitation (CPR) training, with certification every two (2) years, preferred.
ACLS certification, with certification every two (2) years, preferred.
Educational Requirements
Education as required to obtain and keep current ND NP/PA license.
Experience Requirements
Extensive knowledge of established professional medical concepts, principles and practices, required.
Experience in specialized medical areas, preferred.
CPT coding knowledge, preferred.
