Nurse Practitioner - Direct Hire (PACE Program)
Location: National City, CA
Schedule: Monday - Friday, 8am - 5pm
Must be available to work at multiple sites and participate in an on-call panel, including evenings and weekends.
Position Overview:
We are hiring a dedicated and experienced Nurse Practitioner to join a PACE (Program of All-Inclusive Care for the Elderly) team. This role focuses on delivering comprehensive care to an elderly population through a collaborative, interdisciplinary approach.
Key Responsibilities:
Provide primary and preventive care, including diagnosing and managing chronic and acute conditions.
Collaborate with interdisciplinary teams to support holistic care plans.
Refer patients to behavioral health professionals as appropriate.
Supervise nursing staff and support staff in clinical settings.
Ensure timely and thorough documentation within the EMR.
Participate in quality improvement efforts and the development of care protocols.
Provide inpatient and on-call coverage as assigned. Qualifications:
Education:
Minimum: Licensed Nurse Practitioner or Licensed Independent Practitioner
Preferred: Master of Science in Nursing (MSN), National Certification as a Family Nurse Practitioner Experience:
Required: Experience in a community clinic and/or managed care setting
Preferred: Experience working with elderly patients Certifications:
Required: RN license and current CPR certification
Preferred: MSN Other Requirements:
Supervisory experience with nurses
Bilingual (English/Spanish) preferred
Strong oral and written communication skills
Proficient in wound care; ACLS certification preferred
