Job Summary:
The Primary Care Nurse Practitioner will provide comprehensive healthcare services in an outpatient setting. This role involves assessing, diagnosing, and managing a wide range of health conditions for patients across all age groups. The ideal candidate will be committed to delivering exceptional care, focusing on preventive measures and patient education.
Responsibilities:
Conduct comprehensive patient assessments, including physical examinations and health history evaluations.
Diagnose and manage acute and chronic illnesses, coordinating care plans as necessary.
Develop and implement individualized treatment plans based on patient needs and evidence-based practices.
Prescribe medications, therapies, and other interventions in accordance with clinical guidelines and state regulations.
Provide patient education on health promotion, disease prevention, and management of chronic conditions.
Perform routine procedures (e.g., immunizations, screenings, minor office procedures) in a safe and efficient manner.
Collaborate with physicians, specialists, and other healthcare providers to ensure continuity of care.
Maintain accurate and confidential electronic medical records, ensuring compliance with regulatory standards.
Stay current with developments in primary care and participate in continuing education opportunities.
May travel to other CLS clinics as needed.

Current state licensure as a Nurse Practitioner (NP).
Previous experience in an outpatient primary care setting preferred.
Ability to work collaboratively in a multidisciplinary team.
