Job Summary
The Hospice MSW provides emotional support, counseling, and resource coordination to hospice patients and their families. Working collaboratively with the clinical team, the MSW ensures that psychosocial and practical needs are addressed, helping patients maintain quality of life and autonomy throughout their care journey.
Key Responsibilities
Conduct psychosocial assessments of patients and families upon admission and throughout care.
Develop individualized care plans addressing emotional, social, financial, and practical needs.
Provide crisis intervention, grief counseling, and support for anticipatory loss.
Assist patients and families with end-of-life planning, including advance directives and funeral arrangements.
Advocate for patients and connect families with community resources and support services.
Participate in interdisciplinary team meetings and coordinate care with nurses, chaplains, and physicians.
Maintain accurate and timely documentation in compliance with regulatory and organizational standards.
Qualifications
Masters degree in Social Work (MSW) from an accredited school.
Licensed in the state of Texas.
Minimum of one year experience in medical, palliative, or hospice social work preferred.
Strong interpersonal skills with a compassionate and non-judgmental approach.
Valid drivers license and reliable transportation; travel to patient homes required.
Knowledge of Medicare/Medicaid hospice regulations a plus.
