Job Title: Licensed Practical Nurse (LPN) – Long-Term Care Facility

Location: New York (Local Candidates Only)

Shift Options: 7:00 AM – 3:00 PM 3:00 PM – 11:00 PM 11:00 PM – 7:00 AM

Schedule: Full-Time Every Other Weekend Required

Contract Duration: 186 Weeks

Guaranteed Hours: 40 Hours/Week

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our long-term care team. Under the direction of a Registered Professional Nurse, the LPN will provide high-quality supportive and restorative care to an assigned group of residents. This role plays a critical part in enhancing our residents’ quality of life and supporting a team-based approach to care.

Key Responsibilities:

Deliver direct nursing care to residents with complex needs including hygiene, medical treatment, nutrition, mobility, and elimination.

Direct the care performed by Certified Nursing Assistants.

Administer medications (excluding blood/blood products, first IV solutions with meds, IV push meds, central line meds).

Perform IV therapy tasks (if IV-certified) including venipuncture, fluid administration, monitoring, dressing/tubing changes, and IV discontinuation.

Prepare residents for medical procedures and transportation.

Participate in resident admissions and support acclimation to the facility.

Contribute to and implement Person-Centered Care Plans.

Support residents and families with health teaching and communication in line with the Residents' Bill of Rights.

Attend interdisciplinary care planning (ICP) and performance improvement (PI) meetings.

Actively assist during meal times and post-mortem care as needed.

Requirements:

Education & Experience:

Completion of an accredited LPN Program

2+ year of relevant nursing experience (Preferred)

Experience in a Nursing Home or Long-Term Care setting

Licensure & Certification:

Current LPN license in New York State

BLS (American Heart Association) or NY State CPR Certification

Skills:

Strong clinical and technical nursing skills within LPN scope

Excellent communication, interpersonal, and team-building abilities

Ability to maintain a high standard of care under direction and as part of an interdisciplinary team
