Overview
As a PACE Scheduler at our client, you will play a vital role in coordinating participant appointments and ensuring seamless care delivery for older adults. Your responsibilities will include scheduling, communication, documentation, and collaboration with key teams to support the well-being of the seniors we serve.
Key Responsibilities
Coordinate and schedule participant appointments
Facilitate external appointments
Organize new participant orientation & initial assessments
Optimize scheduling across care settings
Collaborate with key teams
Liaison between key departments
Participant communication & follow-up
Manage scheduling conflicts & urgent requests
Maintain accurate documentation
Schedule and Shift Details
Monday - Friday 8a-5p On-site
