Hours: 20.00 per week Position Summary

The Case Manager/Gender Medicine Peer Navigator is to function as a core member of a collaborative care team that involves a multidisciplinary team. They are responsible for supporting mental health care by assisting patients with identified social determinants of health issues. The Case Manager/Gender Medicine Peer Navigator serves as the primary point of contact for transgender and gender-diverse individuals seeking services within the health system. This includes coordinating referrals to appropriate services such as mental health, medical, vocational, housing, social services, financial resources, and gender affirming care.

Essential Duties & Responsibilities

Direct Patient Contact:

Participates in direct care to assess needs and coordinate services

Provides guidance, support and care coordination to comprehensive medical, mental health, and social services for transgender and gender-diverse individuals.

Maintains daily/weekly scheduled appointments in electronic records systems

Facilitates of a variety of trans and gender diverse support groups to foster community and peer connection

Work in collaboration with multidisciplinary team members including external partners

Provide case management services to patients within the Beit Paley outpatient services program

Completes required documentation and paperwork including progress notes and treatment planning in a timely manner in EHR

Participate in our high risk program

Demonstrates cultural competency and commitment to patients including LGBTQ+ community

Maintain case load dependent on need

Provide home visiting or visits in the community to support patient care Minimum Qualifications

Bachelor's degree in social work or a related field

Valid driver's license

Experience working with transgender/gender diverse people

Comfort with patients with mental/behavioral health disorders

Must have excellent writing and communication skills, be highly organized

Must be able to work as a part of a team Preferred Qualifications

Previous experience providing direct patient care is desirable

Knowledge of substance use disorders

Experience with community and home visiting

Knowledge in working with LGBTQ+ Comprehensive Benefits Offered
