Job Summary

The Advanced Practice Provider at Prevea is an advanced practice nurse (NP) or physician assistant (PA) prepared to deliver health care services to patients, families and groups emphasizing the promotion of health and prevention of disease under direct supervision and responsibility of physicians. The scope of responsibilities includes collaborating and coordinating with physicians and other health professionals regarding clinical practice, education, consultation, research and program development.

What you will do

Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries. Including performance of follow up exams if required.

Collaborates with patients to review their health background and determine physiological, emotional, and environmental needs, using this information to advise patients and their families on future health plans and illness prevention.

Provides case management services to assist clients and families with the arrangement for, or refer patients to needed services.

Conducts telephone calls back to patients requiring follow-up care or provide test results. Also provides health counseling and education to patients and their families about their healthcare and any follow-up instructions.

Provides services, which may include the ordering of appropriate diagnostic and laboratory tests, the prescription of pharmacological agents, and other non-pharmacological treatment modalities.

Maintains and reviews patient records, charts, and other pertinent information, documenting all details in the electronic medical record.

Collaborate with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.

Education Qualifications

Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program.

Required Experience Qualifications

1-3 years work experience in a clinical setting within a health care organization Preferred Skills and Abilities

Knowledge of the information and techniques required to diagnose and treat human injuries and diseases, including understanding symptoms, treatment options, drug properties and interactions, and preventive healthcare measures.

Demonstrate organizational skills that allow timely patient appointments and appropriate required follow up.

Presents a courteous and service oriented image through positive and helpful interaction with patients and families, physicians and co-workers.

Excellent interpersonal skills and behaviors

Ability to form work/patient relationships with patients and peers.

Ability to prioritize work and meet changing demands.

Ability to handle and process confidential information.

Fosters teamwork through effective written and verbal communication. Licenses and Certifications

Individual must posses either the licenses/certification details of a Nurse Practitioner or a Physician Assistant.

Nurse Practitioner: Registered nurse in the state of Wisconsin with a certification through American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioner Certification Board (AANPCB). Upon Hire Required

Physician Assistant: Licensed by the state of Wisconsin as a Physician Assistant.

Certification from the National Commission on Certification of Physician Assistant. Upon Hire Required

CPR/Basic Life support for Healthcare Providers certification from approved program. Required Physical Demands
