Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive. We take a comprehensive approach by offering more than 180 integrated programs in: Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. King Gardens is a residential building located at 211 Riverdale Avenue on the corner of Rockaway Avenue in the Brownsville section of Brooklyn. It consists of a six-story elevator building containing 66 residential units, with 65 units of rental housing and one superintendent’s unit. CAMBA, Inc. is the on-site social services provider at King Gardens. Twenty apartments, approximately 30% of the units in the building house chronically homeless single seniors, ages 62 and over, who are diagnosed as SPMI (seriously and persistently mentally ill) or MICA (mentally III and chemically addicted).

What The Case Manager Does:
List the essential functions/job duties of the role.
Maintain professional relationships with clients and adhere to HIPAA regulations.
Follow Universal Precautions and Standard Protocols.
Comply with all relevant security and privacy policies.
Communicate effectively with clients, staff and supervisors
Maintain a caseload of ten tenants (62 years and older) diagnosed with a serious mental illness (SMI) who were formerly homeless.
Maintain and update your assigned charts on daily basis.
Maintain at least two monthly tenant contacts in the office and in the apartment.
Assist tenants in developing services plans and assessments.
Facilitate groups.
Essential functions are specific to the individual position, they are based on the actual work performed, not the ability of the employee.

Minimum Education/Experience Required:
Bachelor’s degree
Other Requirements:
Bachelor’s degree in social work (B.S.W.) or related field and 2 years of relevant experience (Preferred)
Commitment to the mission and values of CAMBA
Knowledge of relevant best practices, regulations and compliance standards required.
Strong communication, organizational and multitasking skills.
Computer proficiency (MS Office – Word, Excel, Power Point and Outlook).
Ability to respond to tenant emergencies or crises and provide acute intervention, de-escalation and or initiate 911 or mobile crisis calls.
As a Mandated Reporter, you must immediately report suspected incidents/cases of abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline at and complete a CAMBA Incident Report
Compensation: $46,350 annually
When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
Status: Full-time (35 hours per week) (Monday-Friday 9AM-5PM)
