Key Responsibilities:

Perform in-home patient assessments and implement care plans

Provide skilled nursing care, including IV therapy, wound care, and disease management

Complete and submit OASIS documentation accurately and on time

Educate patients and caregivers on treatment plans and self-care

Work collaboratively with the healthcare team to ensure the best patient outcomes

Qualifications:

Active RN license in Georgia

Minimum of 2 years of home health experience

Strong proficiency in OASIS documentation required

Current BLS certification (AHA)

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
