Schedule: Monday Friday, 8:00 AM 5:00 PM

Productivity Expectation: Minimum of 30 patients per week

Territory: 50-mile radius around the office

Job Description:

We are seeking an experienced and dedicated Registered Nurse (RN) to join our Home Health team in Scottsdale, PA. In this role, you will provide, coordinate, and direct home nursing care in accordance with physicians orders and agency policies, using the full nursing process.

Key Responsibilities:

Evaluate and assess patients' medical conditions and initiate the appropriate care plan

Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses

Maintain accurate clinical documentation and communication

Collaborate with physicians and interdisciplinary team members

Educate patients and caregivers on care plans and health management

Utilize Homecare Homebase (HCHB) for electronic medical records

Qualifications:

Minimum of 23 years of nursing experience (Home Health experience required)

Current CPR certification

Bachelors degree in Nursing (BSN) preferred
