SUMMARY

The Director, Ambulatory Practice Transformation and Quality Performance is responsible for strategic and operational leadership as it relates to ambulatory and specialty value-based care to include the following: quality performance and preventive care; care management, care coordination and transitions;, patient outreach for gap closure, and NCQA to ensure accreditation as a Patient Centered Medical Home (PCMH). Builds a high-performing team which is empowered and accountable to achieve both department and medical group objectives.

The Director, Ambulatory Practice Transformation and Quality Performance reports dually, to the Senior Vice President, VHC Health Medical Services, and the Vice President, Medical Affairs, VHC Medical Group with indirect reporting to the Associate Vice President, Quality and Patient Safety. This position is a member of the Medical Groups Executive and Operations Council, and is accountable for meeting financial and operational population health, practice transformation, quality performance goals and standards, as well as, managing to an operational budget. This leader will participate in, and at times lead, VHC Health and VHC Health Medical Group, committees, councils, initiatives and projects, that advance the objectives of VHC Health’s, mission, vision, and anchors/values.

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Develop and implement strategic goals related to population health and practice transformation, including setting the direction for strategy and development of process- and outcomes-related key performance indicators (KPIs)

· Responsible for tactical planning that support day-to-day operations of all ambulatory population health, quality performance, care coordination and transitions, care management, outreach, and practice transformation initiatives.

· Directly hire and lead staff to develop a high performing team of clinical and non-clinical professionals that will deliver exceptional support for providers and care for the patient population while meeting population health, quality, operational and financial objectives and targets

· Design an outreach plan and led the team responsible for preventative screenings, chronic care testing, and data collection to close gaps as related to preventative measures, at the federal, state and commercial payer program-level.

· Work collaboratively with our CMS and managed care teams and commercial payer leadership to develop and execute upon KPIs related to population health, quality outcomes, and costs of member care.

· Collaborates regularly with leadership within the health system and medical group to ensure population health, practice transformation, operational and financial priorities are aligned with the health systems objectives.

· Partner/ collaborate with clinical and administrative leaders within service lines and departments for the planning and implementation of change management and process improvement

· Responsible for the medical groups regulatory and payer reporting specific to federal, state and commercial payer population health and value-based care reporting. Additionally, reporting information and data as part of our participation in accountable care organizations, and alternative payment models.

· Produce, and analyze dashboards,/scorecards that allow the health system and medical group measure, understand and improve its quality, operations and cost structure, models and outcomes.

· Responsible for member- and provider-based program design and improvement, in collaboration with subject matter experts within the organization, to improve quality and outcomes of care, improve patient experience and reduce avoidable costs

· As an leader, create, embrace and contribute to our culture of service, specifically around process improvement to add value to the organization

EDUCATION and EXPERIENCE

· Bachelor’s Degree required; Preferred: Master’s Degree (MSN, MBA, MPH or related).

· Preferred: Process Improvement Certification (Lean Six Sigma)

· Preferred: 3+ years’ experience in a director level or higher management position overseeing clinical services and/or provider support services in a health plan, clinically integrated network (CIN), management Services organization (MSO), accountable care organization (ACO), risk-bearing IPA or similar organization, or some combination of the above

KNOWLEDGE, SKILLS and ABILITIES

· Thorough understanding of best practices for population health management, quality, care transitions and management, outreach, and provider practice support

· Exceptional leadership qualities with strong clinical understanding, business acumen, critical thinking, and sound decision-making

· Ability to communicate effectively, verbally and in writing.

· Demonstrated ability to

o Review data analytics reports,

o Develop and implement measurable actions for improvement,

o Review progress towards data-driven performance goals and

o Adjust as needed to meet performance goals

· Demonstrated ability to work with and through others to accomplish specific goals.

· Advanced in use of Office 365, Microsoft Word, Excel, Outlook, etc. Proficiency with care management and/or analytics platforms strongly preferred

· Proficiency with EPIC
