Job Description:
The primary responsibilities of the Assessor are:
Provide substance use Level of Care Assessments via assessing the patient's needs and determine appropriate level of care based on placement criteria established by the American Society of Addiction Medicine (ASAM)
Maintain patient chart via electronic monitoring records with accurate and complete documentation
Abide by regulatory compliance, local, state and federal laws including HIPPA and 42 CFR
Coordinate continuum of care activities for individuals seeking assessment, provide access to available ancillary services, and ensure optimum compliance with external agencies and referral source requirements
Facilitate appropriate funding arrangements
Requirements
Experience providing level of care assessments to SUD and co-occurring population required and/or experience providing screening and assessments per ASAM guidelines.
Candidates will possess a minimum of Masters Level Degree in a related field
Knowledge of funding sources such as Medicare, Medicaid, Commercial/Private insurance
