About the Role

An established healthcare organization is hiring a Clinical Transition Nurse (CTN) in San Diego to guide patients through the hospice onboarding process. This key position ensures personalized, coordinated, and dignified care at one of life's most sensitive stages.

Key Responsibilities

Perform in-depth clinical and psychosocial assessments.

Collaborate with Clinical Teams:?Collaborate with clinical teams by actively sourcing hospice referrals from healthcare providers in the medical facilities.

Develop tailored care plans centered on comfort and symptom management.

Work alongside interdisciplinary teams to ensure seamless, holistic support.

Provide education to patients and caregivers on hospice care and expectations.

Assist with referrals and facilitate admissions when criteria are met.

Lead communication among medical providers, patients, and families.

Complete documentation, including consent and benefits reviews.

Support outreach and community awareness of hospice services.

Qualifications

Graduate of accredited nursing school; BSN preferred.

Valid California RN license and CPR certification.

Hospice experience (1+ years) and 1–2 years in acute or home care required.

Licensed driver with insured, reliable transportation.

Excellent interpersonal and clinical coordination skills.

Holistic/alternative care interest is a bonus.
