Hospice Home Health Registered Nurse (RN) - MUST HAVE RN HOSPICE EXPERIENCE
We are seeking a compassionate, dedicated Hospice Home Health Registered Nurse (RN) to provide end-of-life care to patients in the comfort of their own homes. This role is perfect for a nurse with a passion for hospice care and a desire to make a meaningful impact on the lives of patients and families navigating the challenges of terminal illness. You will provide holistic care focused on comfort, dignity, and quality of life for patients, working closely with an interdisciplinary team to ensure exceptional patient-centered care.
Key Responsibilities:
Patient Assessment & Care: Perform thorough assessments of patients' physical, emotional, and psychosocial needs. Administer medications, wound care, pain management, and other nursing interventions according to care plans.
Care Plan Development: Collaborate with physicians, hospice team members, and families to create, implement, and adjust individualized care plans, ensuring a patient-centered approach focused on comfort and quality of life.
Symptom Management: Assess and manage symptoms related to terminal illness, including pain, nausea, anxiety, and respiratory distress, with a goal to improve patient comfort.
Family Support & Education: Provide emotional support and education to families, helping them understand the patient s condition, care needs, and available resources, while also offering guidance on caregiving and coping strategies.
Palliative Care: Focus on the physical, emotional, and spiritual comfort of patients, ensuring that care addresses the whole person, not just their illness.
Documentation & Compliance: Maintain accurate, up-to-date documentation of patient progress, nursing interventions, and outcomes in accordance with regulatory standards and agency policies.
Collaborative Care: Work as part of an interdisciplinary team, including social workers, chaplains, and other healthcare professionals, to provide holistic care to patients and families.
End-of-Life Care: Provide expert care during the final stages of life, managing comfort and supporting families through the dying process.
On-Call Duties: Participate in an on-call rotation as required, providing after-hours support for patients and families in need of urgent care.
Qualifications:
Licensure: Active, unencumbered Registered Nurse (RN) license in the state of Indiana.
Experience: Minimum of 6 years of nursing experience, with at least 2 years in hospice, palliative care, or home health preferred.
Certifications: BLS (Basic Life Support) certification required. Certification in Hospice and Palliative Nursing (CHPN) is a plus.
Skills: Exceptional communication skills, ability to provide compassionate and empathetic care, strong critical thinking, and problem-solving abilities. Ability to work independently and manage a caseload in a home setting.
