Physical Therapist

Seeking a PRN Nurse Practitioner for home risk assessments, including evaluations of medical history, social factors, and risks during visits up to one hour.

Open to new grads with required RN and NP licensure, certification, and at least 1 year of NP experience.

Conduct assessments in home, SNF, and community settings, collaborating with healthcare teams and providing patient education.

Flexible scheduling including days, evenings, nights, weekends, with compensation from $2,000 to $10,000/month based on completed assessments.

Must have reliable transportation, valid driver’s license, and travel ability to meet patients where they live.

Benefits include mileage reimbursement, 401(k), and voluntary benefits; PRN status means no benefits apart from those listed.

Required credentials: Master’s degree or equivalent, current RN and NP licensure, board certification, and BLS/ACLS.
