New Beginnings Reentry Services – Adult Reentry Day Center

Position Summary:

New Beginnings Reentry Services is seeking a dedicated and compassionate Case Manager to provide critical support for adults involved in the reentry process. The Case Manager will play an essential role in guiding individuals through the intake process, ensuring they access key services such as MassHealth/insurance enrollment, housing resources, and workforce development programs. This position will require close collaboration with community partners, including MassHire, local housing agencies, and other service providers, to ensure clients receive comprehensive support for their successful reintegration into society.

Key Responsibilities:

• Intake Process and MassHealth/Insurance Enrollment:

Assist clients through the intake process, including gathering necessary documentation and completing applications for MassHealth and other insurance programs. The Case Manager will ensure that clients are connected to appropriate health services and treatment plans through the MassHealth system and/or other insurance providers.

• Housing Referrals and Support:

Provide critical support in securing stable housing for clients by connecting them with transitional housing, halfway houses, and permanent supportive housing options. The Case Manager will work closely with local housing authorities and housing programs to facilitate referrals and ensure clients are placed in appropriate housing based on their needs and eligibility.

• Collaboration with MassHire and Employment Resources:

Work directly with MassHire and other local workforce development organizations to assist clients in securing employment opportunities. The Case Manager will help clients access job training, resume building, interview preparation, and job placement services to ensure successful employment outcomes.

• Coordination of Social Services and Benefits:

Assist clients in applying for and accessing public benefits, including food assistance (SNAP), cash assistance (TAFDC), and other public health and welfare programs. The Case Manager will coordinate with community resources to help clients obtain any benefits or financial support that is available.

• Development of Individualized Case Plans:

Create personalized case plans that address each client's unique needs, including mental health, substance use recovery, housing, employment, legal support, and social services. These plans will outline specific goals and action steps to help clients move toward long-term stability and success.

• Resource Coordination and Referrals:

Coordinate referrals to a wide range of services, including mental health and addiction treatment programs, legal assistance, and educational resources. The Case Manager will ensure that clients are connected to services that align with their case plans and offer follow-up support to ensure service engagement.

• Monitoring and Advocacy:

Monitor client progress and assist with overcoming barriers to service access. This includes regular check-ins to assess progress, adjusting case plans as necessary, and advocating on behalf of clients to ensure they receive the services and support they need.

• Collaboration with Community Partners:

Build and maintain strong relationships with local service providers, including MassHire, housing authorities, health providers, legal aid organizations, and other community-based programs. The Case Manager will coordinate with these partners to create a comprehensive support network for clients.

• Crisis Intervention and Support:

Provide crisis intervention services when necessary, including addressing housing instability, substance use relapse, or mental health crises. The Case Manager will act as a point of contact for urgent issues, helping clients access emergency resources and services.

Additional Responsibilities:

• Documentation and Reporting:

Maintain thorough and accurate documentation of all client interactions, case plans, referrals, and progress notes. This includes tracking outcomes related to housing placement, MassHealth/insurance enrollment, employment, and overall stability. Documentation will also be used for internal reporting and evaluation.

• Client Education and Empowerment:

Educate clients about available resources, self-advocacy, and their rights within the service systems. Empower clients to take an active role in their case plans and reentry process.

• Program Participation:

Participate in staff meetings, case review sessions, and professional development opportunities to ensure the best possible service delivery to clients. Keep up-to-date with changes in state and federal benefits programs, housing options, and workforce development initiatives.

• Compliance with Policies and Procedures:

Follow New Beginnings Reentry Services’ policies and procedures, including those related to confidentiality, ethical standards, and service delivery. Ensure that services are provided in accordance with agency guidelines and best practices.

Qualifications:

• Experience in Case Management or Social Services:

At least 2 years of experience in case management or social services, particularly in working with individuals involved in the criminal justice system, mental health, substance abuse, or homelessness.

• Knowledge of MassHealth and Public Benefits Systems:

Familiarity with MassHealth and other public benefits programs, including eligibility requirements and application processes. Experience helping clients navigate health insurance enrollment and public assistance programs is strongly preferred.

• Understanding of Housing Resources:

Knowledge of housing resources and programs available to individuals in reentry, including transitional housing, sober living, and permanent supportive housing. Experience in facilitating housing referrals is preferred.

• Collaboration with Workforce Development:

Experience working with MassHire or similar workforce development programs, including job readiness training, employment resources, and career counseling.

• Strong Communication Skills:

Excellent verbal and written communication skills, with the ability to effectively engage clients, community partners, service providers, and legal professionals.

• Cultural Competency and Client-Centered Approach:

Ability to work with diverse populations and demonstrate cultural awareness, flexibility, and a non-judgmental approach. Must be committed to person-centered, trauma-informed care.

• Ability to Work Independently and as Part of a Team:

Strong organizational and problem-solving skills, with the ability to manage a caseload independently while collaborating effectively with multidisciplinary teams.

• Crisis Management and Problem-Solving Skills:

Ability to manage crises and sensitive situations with empathy, patience, and professional judgment.

• Education and Training:

o Bachelor’s degree in social work, psychology, criminal justice, or a related field (preferred).

o Completion of Crisis Intervention Training (CIT) and Coordinated Entry training (or willingness to complete upon hire).

• Travel Requirements:

A valid driver’s license is preferred, as local travel may be necessary to meet clients, attend meetings, or visit service providers.

Accountability:

The Case Manager is accountable to the client, the agency’s policies and procedures, and the overall success of the Adult Reentry Day Center’s reentry programs. The Case Manager will work closely with the leadership team at New Beginnings Reentry Services to ensure that services meet client needs and organizational goals.
