CARE NAVIGATOR

Adagio Health Inc is looking to hire one (1) full-time Care Coordinator Specialist for its Beaver County (Hopewell) office.

We are looking for compassionate and driven individuals to join our team as a Care Coordinator Specialist. This role is integral to our mission of providing comprehensive support and care to a diverse range of clients, including those with specific needs such as HIV, veterans, and others requiring specialized services.

RESPONSIBILITIES

Client Assessment and Support: Conduct thorough assessments of client needs, focusing on behavioral health, food insecurity, transportation, housing, and other essential community services.

Knowledge Development: Gain in-depth knowledge of available community support services and programs, including the referral process for each.

Veteran-Specific Expertise: Become adept in military benefits, documentation, and health insurance navigation to support our veteran community effectively.

Referral Management: Provide well-coordinated referrals, both internally and externally, ensuring a seamless transition for clients.

Barrier Identification and Resolution: Assist clients in identifying and overcoming barriers to service access, utilizing local resources and supports to optimize care.

Educational Support: Offer education and resources to clients as needed, enhancing their understanding and engagement in their own care.

Documentation and Data Management: Meticulously document all client interactions and data, participating in data analysis and reporting.

Quality Improvement Participation: Engage in initiatives aimed at enhancing the quality of services provided.

Collaboration and Outreach: Work closely with clinical and program staff, maintaining open communication lines. Conduct outreach to build and maintain relationships with community partners.

Veteran Mental Health First Aid (MHFA): Obtain and provide Veteran MHFA to partners and veteran audiences.

Follow-Up: Ensure continuous care by providing consistent follow-ups with patients, clients, and partners.

Teamwork: Collaborate with various community service providers, including clinical, nutrition, and ancillary support services.

Technology Utilization: Employ virtual/tele-health and texting program equipment for remote client interactions.

Additional Responsibilities: Be adaptable to perform other duties as assigned.

QUALIFICATIONS FOR A CARE COORDINATOR SPECIALIST

Accredited degree in Social Work, Public Health or related field. Experience in case management/care planning, customer service, and enrollment process for medical insurance products, preferred. Must be able to comfortably work with Microsoft office products. Must have excellent communication skills. Some regional travel required.
