We are seeking an experienced Hospice Case manager to join our team of medical professionals.

Position Overview:

The successful candidate will be responsible for providing quality care to patients in hospice settings.

The ideal candidate should have be experienced in the Hospice Industry, communication skills, and the ability to work independently and as part of a team.

This is an excellent opportunity for an experienced nurse make a difference in the lives of patients and their families.

Key Responsibilities:

Coordinate and provide care that is safe, timely, effective, efficient, and client-centered

Handles and manages case assignments

Facilitate multiple care aspects (case coordination, information sharing, etc.)

Aid patients in making informed decisions by acting as their advocate

Develop effective working relations and cooperate with the clinical team throughout the case management process

Adhere to professional standards as outline by protocols, rules and regulations

Completing initial, comprehensive, and ongoing assessments for patients and families to determine hospice needs
