Position Summary

We are seeking a compassionate, organized individual to become the Community Health Worker within the Refugee Health Assessment Program. This unique opportunity allows you to work with the newly arrived refugees, asylees, and special visa holders looking to make the United States their new home. This position will require someone who is fluent in a language other than English. The Community Health Worker will assist patients in accessing the variety of social and medical services they need to maintain their health.

Key Functions & Responsibilities

Provide education on preventive care, health promotion, and disease management to individuals and families within the target population.

Assist individuals in navigating the healthcare system, scheduling appointments, and accessing necessary resources and services.

Support the clinical team by assisting in screening, assessing, and completing other health-related tasks as needed.

Establish trusting relationships with patients and their families and provide general support and encouragement.

Conduct intake interviews with patients, including enrolling patients into, and/or referring them to, various health, assistance, and social service programs.

Follow-up with patients, by phone, home visits, and visits to other settings where patients can be found.

Assist patients with completing applications and registration forms.

Conduct and verify eligibility determination, enrollment, and follow-up with uninsured patients.

Help patients connect with transportation resources and give appointment reminders in special circumstances. Transporting patients is strictly prohibited.

Be knowledgeable about community resources appropriate to the needs of patients/families.

Act as a patient advocate and liaison between the patient/family and community service agencies (i.e., schools, Department of Human Services, Resettlement Agencies, hospitals, support groups, etc.).

Record patient care management information in the EMR (training provided) and other software no later than 24 hours after patient contact.

Attend regular staff meetings, training, and other meetings as requested.

This position may require travel between CCI sites to provide additional coverage when needed.

Other duties as assigned.

Minimum Requirements

High School graduate, or equivalent

Current CHW certification from an accredited program, highly desired.

Current BLS/CPR certification required.

Previous experience as a Community Health Worker, preferably working with diverse populations.

Cultural competence and ability to work effectively with individuals from various cultural backgrounds.

Excellent communication and interpersonal skills.

Proficiency in languages commonly spoken within the refugee population (i.e., Pashtu, Dari, Haitian-Creole, Arabic, and Dinka) is required.

Familiarity with the local healthcare system and resources.

Ability to work flexible hours, including evenings and weekends, to accommodate the community's needs.
