We are seeking an Application Analyst for our Population Health - Epic Care Management team. This covers workflows for inpatient, outpatient, and transitional care management.

Responsibilities:

Install new applications, new components, and upgrade existing applications. Configure application settings.

Provide go-live and rollout support

Interface with users/clients across the Montefiore Health System. Support users on new applications, modules, and functionality

Perform analysis of necessary application modifications and work with the appropriate Montefiore IT associate or the vendor to coordinate the modification

Determine how a new system, upgrade, or new component can best meet the needs of the users:

Analyze business/clinical needs and requirements

Interpret end user requirements, by fully discussing potential options, and help client move towards optimal solution

Evaluate the operational workflow that is applied to the system

Create system specifications from user requirements

Create test plans and test new systems, version upgrades, and any system modifications

Document all outcomes of testing

Application implementation, troubleshooting, and support

Provide day-to-day operational support, advanced application troubleshooting support, coordinate modifications and updates, recommend improvements & increase efficiencies and provide pro-active monitoring

Provide 24 x 7 on-call coverage for user support on a rotating basis

On an as-needed basis, provide on-site support within the Montefiore Medical Center hospitals and/or its’ affiliates (on-site, rotational support)

Required:

Bachelor’s degree or equivalent experience

Epic certification in Healthy Planet and/or Compass Rose, ClinDoc or Clinical Case Management (Strong Consideration will be given to candidates that can demonstrate ability to pass EPIC Certification in combination with relevant skills/experience)

2+ years of related clinical or business experience related to population health/care management

Application implementation, troubleshooting, testing, and support experience

Strong desktop skills including Word, Excel, PowerPoint, Visio

Critical thinking skills

Strong problem solving and analytical skills

Strong interpersonal skills with customers, vendors, peers, and management

Eager to learn, exceptional listening skills

Excellent written and verbal communication skills

Excellent follow up skills, documentation skills and organizational skills

Must be adaptable in a rapidly changing environment with competing priorities

Meeting all requirements for Epic certification is a condition of employment
