THIS IS NOT A REMOTE POSITION. PATIENTS ARE LOCATED IN ROCKLAND AND ORANGE, NY.

United Hospice has a new and exciting opportunity for a compassionate and experienced Admissions Nurse with Hospice experience, to join our talented team. As we prepare to expand our services to Orange County, we are seeking a Rockland County-based Admissions Nurse (RN) and an Orange County-based Admissions Nurse (RN).

It is essential our employees have an understanding and commitment to the hospice philosophy of care. Working with a diverse patient population, you must be able to communicate effectively, while sustaining a caring, compassionate and professional disposition. The successful candidate will have strong clinical assessment skills as well as excellent written and verbal communication.

As an Admissions Nurse, you will conduct the initial visit with patients and their families in their homes, inpatient facilities or in our United Hospice office to describe our comprehensive services. This will include but is not limited to explaining Hospices’ philosophy, services, and Advance Directives. You will assess the patient and family’s needs for an array of hospice services. You provide patient family counseling and education when appropriate.

A full admission consists of performing physical and pain assessments and obtaining demographic, medical, and historical information. This includes obtaining all necessary consent and authorizations in order to prepare the initial nursing care plan.

For patients who require an admission while inpatient, you will work with the hospital discharge planning team to effectively coordinate a seamless discharge to hospice.

Qualifications:

§ Registered Professional Nurse (RN) with a current NYS License.

§ The qualified candidate must have at least two years of recent clinical nursing experience in either hospice or hospital-based med/surg nursing, with electronic medical record competence. Admission and end-of-life care experience preferred.

§ Experience in home care, hospice and/or oncology preferred; and

§ Bilingual Spanish-English a plus with additional compensation at hire.

Salary range for this position is $74,000 - $105,000
