Job Description
The Case Manager helps to facilitate many aspects of the client’s life. We are looking for someone that wants to be part of our team. Decisions for Life works with adults with dementia, chronic mental illness, intellectual disabilities and developmental disabilities. Some of those areas will involve:
Interacting with clients to assist in meeting their needs.
Making professional decisions and recommendations that impact the person served.
Providing or directing the preparation of necessary records and reports.
Serving as a member of an interdisciplinary treatment team.
Gathering and evaluating information regarding clients or programs.
Documenting case work entries into the internal case management system in a timely manner.
Duties include, but are not limited to:
Traveling at least quarterly to see clients
Reviewing their living environment
Ensuring they have their basic necessities as finances allow
Making sure they are accessing all available services
Communicating with family and friends
Scheduling and attending medical and dental appointments
Implementing and monitoring medical recommendations that arise from those appointments
Visiting clients, reviewing facility records and attending care conferences and care planning meetings
Purchasing items, clothing, and other items as needed
Assisting clients with accessing the community resources
Supporting clients in achieving and maintaining their maximum independence
Documenting work in a timely fashion
Attending staff meeting as scheduled
Other duties as assigned
The Case Manager is responsible to the head of Decisions for Life, Sherri McLerran.
