Licensed Practical Nurse (LPN)-Hospice Nurse Community Nurse Position Parker Jewish Institute Comprehensive Community Hospice is seeking dedicated and compassionate Full Time LPN-Hospice Nurses to join our team.

Community and Inpatient Hospice at Parker Jewish Institute provides comfort, relief, and peace of mind to individuals with advanced or life-limiting illness and their families.

We are an intimate and unique not-for-profit program that provides personalized care to ensure the comfort, enhance the quality of life, and preserve the dignity of our patients.

Your role will be to ensure these needs are met by providing direct care, ongoing symptom management, documented coordination of care and family support for our patients.

Here are some additional highlights you'll want to know about the candidates we seek: Current New York State license as a licensed practical nurse (LPN) 1+ years of experience as an LPN in a home care, hospice, med surge or equivalent role Previous community LPN experience in hospice or home care a plus but not required-we provide training and mentorship!

Knowledge of care coordination a plus Able to use computer technology and electronic medical records in the field Access to reliable transportation and willing to conduct home visits Vaccinated against COVID-19
