Salary: $110 - $115k

Envision Healthcare at Home was established in 2005 by six healthcare professionals who wanted to make a difference in their community.

Envision is currently looking for an empathetic and service-oriented RN for our Hospice Clinical Manager (or Patient Care Manager) for our patients in Kitsap County and for King County. The Hospice Clinical Manager will oversee nursing staff in the appointed area and will operate with staff in the field. This position reports directly to the Director of Clinical Services.

The person in this role will be Supporting 4-5 RN case managers, will be mentoring, and teaching the RNCMs. This person may at times carry a small caseload of a couple of patients during times of growth. They would also be responsible for doing chart reviews, and helping lead the IDT meeting.

Responsibilities:

Participates in developing standards and objectives for Hospice, which ensure safe and therapeutically effective service to patients and families.

This person will have a joint responsibility with the Administrator and Director of Clinical Services to see that standards are met.

Manages the census and staffing for the region.

Proficient in admissions and case management.

Initially markets to local referral sources and meets with families interested in hospice services.

Conducts initial assessments and visits to patients until appropriate personnel are hired.

Consult with the State Administrator and DCS to determine a staffing pattern that will accomplish stated objectives and promote the maximum level of utilization of personnel.

Is responsible for hiring, evaluating, and terminating service personnel under the guidance of Senior Leadership.

Participates in planning for the orientation of new employees. Conducts selected orientation classes.

Plans and arranges for consultation needs of staff; prepares and maintains current policies and procedures that meet Medicare, Medicaid, accreditation standards, state and federal laws, etc. and implements; revises concurrently.

Displays a willingness to support the policies and procedures and uses appropriate channels for change of such policies; establishes criteria and procedures for selection, promotion, and the termination of employment of service personnel.

Participates in establishing the functions and qualifications for each service position.

Coordinates interdisciplinary team services.

Assures ongoing assessment of patient/family needs and implementation of interdisciplinary group plan of care.

Provide individual or group support concerning job-related stress or issues.

Organizes Hospice to delineate and delegate authority, functional responsibility, lines of relationship, and communication to provide safe and therapeutically effective service.

Participates in coordinating Hospice services with services of other community agencies.

Participated in the evaluation of agency performance through the annual evaluation process, quality assurance, performance program, and periodic review of operations.

Establish and maintain working relationships with key line, contracts, marketing, and clinical staff to ensure a thorough understanding of company services.

Develop and implement effective clinical and business management systems, oversee the audits of client charts, client satisfaction surveys, and all service delivery within the region by conducting routine program audits, as assigned.

Qualifications:

Registered Nursing degree required in the state of Employment.

Must have at least 1-2 years’ experience working directly for a Hospice Company or Agency.

Minimum of at least 2 years supervisory/management experience required.

Proven track record of clinical management, education, and management of healthcare staff

Outstanding leadership and managerial skills, Ability to work autonomously, and Experience in building a team.

Strong organizational, interpersonal, and communication skills.

Understanding hospice care and the services provided to patients, families, and caregivers through an interdisciplinary team is strongly preferred.

History of success in revenue growth and the development and execution of market planning

History of compliance with accepted professional standards and practices and state/local administrative licensure requirements
