Summary:

We are seeking an exceptional Healthcare Project Manager to lead the charge in revolutionizing our dynamic healthcare network. As a key player in our team, you will drive forward-thinking initiatives, spearhead business development strategies, and ensure unparalleled performance and growth.

Essential Duties and Responsibilities:

directs and oversees multiple high-impact population health initiatives concurrently, including but not limited to clinical initiatives such as AWV, TCM, and COPD management.

business development initiatives aimed at forging strong alliances with medical facilities, positioning our Accountable Care Organizations (ACOs) as preferred partners. Leverage these relationships to drive growth opportunities, expand our network, and optimize patient-centered care delivery models for sustainable success.

and collaborates with esteemed physicians and providers, serving as a catalyst for change in healthcare delivery.

seamless patient flow management to optimize customer satisfaction and clinical outcomes.

strict adherence to HIPAA, confidentiality, and privacy laws, ensuring utmost protection of patient information.

Qualifications:

leadership experience in driving transformative change in healthcare settings.

track record of self-starting initiative and resourcefulness, coupled with exceptional interpersonal skills to engage stakeholders at all levels.

understanding of provider communities and extensive experience with Electronic Medical Record (EMR) and Health Information Management (HIM) systems.

in Excel, Word, and PowerPoint presentations within a business context.

emotional intelligence and engagement, coupled with progressive operational experience within medical centers, clinical groups, or hospitals.

grasp of Population Health Strategy coupled with the ability to excel in a matrix management environment.

Degree, preferably in Business Administration, Healthcare Administration, or related field.
