Title: Population Health Care Coordinator

Program: HealthPoint

Beginning Salary: Based on Experience

Travel: 5%

Reports to: Director of Quality & Population Health

Duties: Under the general supervision of the Population Health Manager, this position is responsible for working effectively with other members of the Care Manager team to provide high quality, effective care management to HealthPoint patients. Care Coordinators facilitate conversations between interdisciplinary Care Teams (including Care Manager, Care Coordinators, primary care physicians, and additional health care providers) and expedite client services referrals.

Population Health Coordinator serves as a member of the interdisciplinary care team and offers innovative, evidence-based health care support to improve health outcomes for HealthPoint patients. The Population Health Care Coordinator coordinates health center activities and helps to plan, organize, and distribute resources to meet patient care needs. This role will work collaboratively with health center staff to proactively meet patient care needs, engage patients into health care services, address barriers to access, and facilitate connection to needed services and community resources. The Population Health Coordinator will play a critical role in patient health care management to enhance quality, reduce cost and improve health outcomes across all patient populations. The individual in this role is expected to incorporate continuous process improvement into daily work, using critical thinking and problem-solving skills to address opportunities for improvement.

Education:

Population Health Care Coordinator Required:

Successful completion of an accredited Medical Assistant program

Work Experience:

Population Health Care Coordinator Required:

3 years of clinical experience in a health care setting, knowledge of community resources required and working knowledge of provision of health care in a variety of settings.

Licenses, Certificates or Registration:

Population Health Care Coordinator Required:

CPR Certification; Must possess valid state Driver's license and automobile insurance with reliable transportation.
