Qualifications:

Required:

- Master's degree or higher

- Valid Nurse Practitioner (NP) license in Massachusetts (MA)

- Valid Physician Assistant-Certified (PA-C) license in Massachusetts (MA)

- 2+ years of experience in Nursing Primary Care

- Familiarity with Work Relative Value Units (RVUs)

Preferred:

- Experience in a fee-for-service or Value-Based Care (VBC) setting

Job Summary:

The primary role of the facility-based Advanced Practice Provider (APP) is to provide Provider-level direct patient care services within post-acute care settings.

Key Responsibilities:

- Embrace our company's care philosophy to enhance each patient's clinical status and quality of life.

- Conduct Annual Wellness Visits and other clinical assessments as needed.

- Provide ongoing evaluation and treatment of chronic illnesses.

- Collaborate with physicians and other healthcare professionals for comprehensive medical management.

- Maintain communication with families and attend conferences when possible.

Qualifications:

- Must hold a valid nurse practitioner or physician assistant license and be board-certified.

- Bachelor's and Master's degrees in Nursing from accredited institutions.

- Nursing experience is required, with some preference for Nurse Practitioner/Provider experience.

- Strong leadership, interpersonal, and problem-solving skills are preferred.

- Excellent written and oral communication skills are necessary.

- Punctuality, organizational skills, and teamwork abilities are essential.
